The first thing to consider is overuse.
I would try to look for trigger points.
In the quads and hamstrings.

That's the situation for me - permanent life style changes and prescribed a multiple treatment/multiple exercise approach. I've never had sciatica, but my lower back was very fragile about 15 years ago.No easy fixes. Whatever you choose to do it has to become a life style change, something you do consistently for the rest of your days.
