You guys do anything special on these flat decks to keep the rail thickness down when ordering custom?

Yes. Go way thinner than you think is normal. My flat-deck twin is 2.19, which sounds crazy but it seems equivalent to something like 2 3/8 or 2 7/16. The thickness is carried out to the rail with almost zero foiling. So even going close to normal could result in a pretty chunky board. But I would check with the crew at Rusty.

I was trying to keep around 28 liters

at 5’5/20, that put me at 2.12 thick but that seemed crazy low to me so I bumped up to 2.15 which came out to 28.7 liters

for comparison, I have a CI black and white in stock 5’11 which purports to have a flat deck but is 2.25 thick

the deck on the CI is not as flat as on this Rusty

the rusty is flat all the way to the rails

so definitely go thinner than you think

I likey! those would be pretty much my dimensions as well. Good grief, that's a chunky nose though. I know that's probably the way it has to be, but just not used to seeing noses foiled that way on pointy boards.
