Sciatic foramen are lateral passages from the spine through the pelvis to the hip joints. Four segments of sciatic nerve and four segments of piriformis muscle pass through them. An individual patient's configuration of the nerves and muscles determine whether the swelling pulls up on the leg nerve or down on the spine nerve. Pulling down on the spine nerves causes lateral column deformation against the disc at the L4-L5 stack.

Humans evolved our upright gait by escaping land predators by running into cold water. Therefore our foramen only needed to be large enough to accommodate cold, shrunken muscles. That's why these problems are so very common in land-adapted humans. Ice packs help along with the tennis ball massage.

Surfing last summer during a hurricane swell with my 59 year old friend who still rips, I asked him how he keeps so amped up, he said he does this foundation training video (or the original one by the same guy) every morning and he swears it helped eliminate his back pain. Since then I've been doing it in the morning before work followed by a couple sets of push ups. almost every day for months now, seems to really help with back issues. It is only 12 minutes and pretty easy to do.
I actually tried it. I think it will be a good warm up for my bodyweight and barbell strength work outs. It feels good after you do it. I have to have the video on. I don't think I could remember all the prompts. Maybe if I keep doing it.

not flat on the floor, but like down on one knee

Ok, not to make this all about me, but
a few hours ago i wasn’t able to walk a few feet
but I just finished shoveling about 4 inches of fairly heavy snow and feel almost fine

i took a muscle relaxer, which I assume helped
but I’m pretty sure there’s nothing structural or injury wise ailing me
i just totally fucking nuts

@manbearpig how are you doing? And have you at least started the book?

I have not started the book (I thought it was always referred to as the fucking book?).

pretty laid up thanks to Covid. Have been stretching a little, I think improvement has been primarily from the prolonged rest I’m getting. So I guess that’s a silver lining to to the COVID.

There is fight, flight and freeze response to danger.

Your nervous system gets to decide.

In that situation it would likely choose flight.

In a situation like bending over unbraced it might choose to spasm: freeze. It's positions of the body that don't feel safe

I do this every day, as well. Either as a warm up or for maintenance on the days I don't lift. It's especially helpful recovering from back strains when transitioning back to a barbell. Still, it's no substitute for a proper dead lift.

Saw surgeon yesterday to talk about surgery and sadly I am not a candidate for disk replacement. Facets are f^cked as well. 5-7 years out in the USA. Recommended PRP.

Crazy to be sad I cant get back surgery

I do split squats, just started doing Jefferson curls, YTWLs (dumbbell reverse fly), skull crushers… just a few I glimpsed.

In spite of all I do, I still get a sore lower back after surfing (as well as very sore neck and shoulder…only the left, the right is fine:shrug:). I’m just kind of resigned to it. Like that saying, growing old isn’t for pussies.

PT = stretches you can find on the internet so IMO a waste of time and $

for temporary possibly even placebo leves of pain relief get a tens unit. ice for 15/heat for 15. then tens unti for 20 minutes. then just work on your posture, stretch your hips and hammies thats all you can really do.

ive been to amilllion back doctors at this point - they all say there is nothing wrong with my back but these are the things that help me manage it.

PT is a lot more than “stretching” … there’s plenty a crap PTs out there, but the good ones are worth their weight in gold.

Example: I broke my elbow in multiple places about 10 yrs ago. Took a very long time to heal - I ended up w a LOT muscle atrophy and loss of range of motion. Without physical therapy I’d have been fucked. YT vids would have been about as useful as rubbing peanut butter on my elbow.

Example 2: replaced cervical discs and shoulder muscle atrophy, example 3: LB and hip issues.

Same deal PT was a game changer for those. NO stretching involved


I’d stay away from “big box” operations, you want a small outfit, like a sole practitioner, that’s vested in why they’re doing

What did the PT do? I'm assuming targeted exercises. Did they prescribe a comprehensive preventative training regimen for after you regained ROM and some strength?
