I would say this is an aspect of lack of appropriate healthcare

Very few chose to be homeless in any meaningful sense

They are generally mentally ill

Correct.

Mental illness and substance abuse go hand in hand.

For instance, schizophrenics smoke cigarettes at a rate 3x higher then the rest of the population. The nicotine medicates their disease.

That said, something like 60% of the homeless population are actively addicted to drugs or alcohol. These addictions make it impossible to become employed or pay rent, etc. Further, the individual has to choose getting clean - it's their choice entirely.

We need creative legislation to deal with these individuals and state mental institutions to corral them - for their own well being and the well-being of our communities.

Harmful drugs are just one vice. If you survive the first time, why can't you do it again? The high is worth it to some.

S&M pain is another. Many levels of harm and pain are tolerated.
